This Note is to thank all your staff for the very professional care that they provided to me during my procedure on Friday, December 30, 2005 at 10:00 a.m.

The front desk staff explained everything and were very courteous in answering my questions. The technicians and staff in the back provided for my comfort beyond my expectations. Everything was explained to me in advance of the procedure.

I am sorry that I did not get the opportunity to complete the “Patient Satisfaction Survey” that was in the waiting room. I decided to write this memo since I did not complete the survey.

As a health care professional myself, I can honestly say that I was treated very professionally by all your staff from the first person I spoke with to the last person who gave me my films.
